Launches Healthy Feet Foot
Care Program:
We
are pleased to introduce our new foot care program which we have named
"Healthy Feet." The goal of this program
is to provide our clients, especially seniors, with the opportunity
to receive foot care in their home. On the initial visit we will
do a basic foot soak and massage and a skilled nurse will evaluate the
clinical status of the feet and also lower extremities. If problem
areas are identified then a determination will be made as to the next
step, which could include a home visit by a podiatrist or the scheduling
of additional services by a home care aide or skilled nurse on a fee-for-service
basis. If certain specific criteria are met, then the primary
care physician may be contacted to discuss the possibility of services
being provided under the Medicare home health benefit. Please

We
are pleased to announce that Kathy Payne, LVN, is starting her 5th year
as Wellness Coordinator. Kathy has extensive experience in diagnostic
testing and interpretation of test results. Under Kathy's direction,
Calvert offers the following tests for early detection and prevention
of health problems:

Calvert
Participates in Texas Medical Foundation Collaborative Program
Calvert is participating in the Texas Medical Foundation (TMF) Home
Health Collaborative Program. This is a 15-month program in collaboration
with the Centers for Medicare and Medicaid Services (CMS). CMS has established
as a national priority for home health agencies to decrease the Acute
Care Hospitalization of Medicare patients. In addition, Calvert will
also focus on outcomes related to Improvement in Dyspnea and Pain Interfering
with Activity. Calvert is pleased to be working with TMF to improve
patient outcomes.
Calvert Participates in National Home Health Quality Improvement Campaign
The Home Health Quality Improvement National Campaign 2007 is a grassroots collaborative quality improvement effort among the home health community, healthcare leaders, and
Quality Improvement Organizations. The goal of the campaign is for home care patients to remain in their home settings, resulting in fewer avoidable hospitalizations.
Home health agencies participating in the campaign will work to reduce the number of avoidable hospitalizations while improving clinical outcomes and patient satisfaction.
